Improving Patient Education

Kidney Pathways is a new, online, individualized educational approach developed by the National Kidney Foundation. Simply by answering a few questions about their health, anyone who accesses the system will be presented with an individualized education program, tailored to them and their kidney health information needs. In Kidney Pathways, learning topics are dynamically delivered and aligned to each learner’s kidney health status and the presence of risk factors like diabetes and hypertension (high blood pressure).

Learners are asked to complete a simple survey about their health. Do they have diabetes, hypertension, or a diagnosis of CKD? Do they have family members with a kidney transplant or on dialysis? Do they have access to CKD laboratory findings: estimated glomerular filtration rate (eGFR - assessing kidney function) or urine albumin-creatinine ratio (UACR - assessing kidney damage)? What is their weight and height? Once these questions are answered, Kidney Pathways automatically determines what pathway of learning is most appropriate given the information that has been reported.
